Rules

For your ballot to be counted and to be considered for the grand prize, you must complete your ballot entirely (with entries for at least half the categories) and include your name, address, phone number and e-mail. Winners will be selected from a random drawing of completed ballots. No photocopies will be accepted. Ballots without names and phone numbers will not be counted. Only one entry, in any form, per name or address. Votes for national chains may be tabulated and awarded separately. Ballots will be selected at random to be verified by phone.

Entries must be postmarked no later than Jan. 4, 2010.
